Student Registration Information

The forms below can be completed and submitted electronically, along with copies of all necessary documentation, to the District Registrar. Please call her at 518-957-6004 if you have any questions. Email is: student.registration@plattscsd.org

PLEASE NOTE: The Registration form must be COMPLETED and submitted along with the following COMPLETED FORMS: Elementary Health History Forms and Home Language Questionnaire. Don’t forget to submit two (2) proofs of residency, your child’s birth certificate, health physical and immunizations from the doctor’s office, and custody forms (if applicable).To protect you and your child’s personal information, please DOWNLOAD all forms to your computer to complete and sign digitally. Then proceed to attach to an email as instructed to submit. Browsers will not allow digital signatures to be applied. All forms will force the execution of downloading to your computer when clicked.

All forms are best viewed in Adobe Acrobat Reader DC (download here). To opening Adobe Acrobat Reader DC and not your default browser on your computer to fill out and sign accordingly: 1. Right click your document in your saved location. 2. Select “Open With…” and 3. Choose “Adobe Acrobat Reader DC” in the proceeding window that appears or navigate to Other Applications/Programs to find “Adobe Acrobat Reader DC” after installation. You may choose at this step to also make sure all PDF documents open with “Adobe Acrobat Reader DC” and not an alternative application/program before closing out of the selection window.
